Darin M. Camarena Health Centers, Inc. (DMCHC) recommends appointments are made before you visit our health centers. You can make appointments by coming into the health center or by phone. Health center hours vary by location. See clinic hours information by location in left navigation area.

DMCHC accepts same-day appointments for new and established patients for toothaches, cracked teeth, broken dentures any type and other emergencies. All patients will be triaged and seen as determined by the Dental Provider.

For your first visit to DMCHC, bring proper identification, this includes a valid driver’s license, birth certificate or other form of official identification. Always bring your Medi-Cal card, Medicare card or private insurance card to each and every visit to the center.

If you are without insurance, to qualify for Sliding Fee Scale, bring a pay stub, an unemployment check stub, a recent tax return or a letter from your employer in order to prove total family income and family size.

DMCHC accepts Medi-Cal , Medicare, and most private insurance. Our friendly health center staff will get you in touch with Medi-Cal outreach workers and representatives from Healthy Families to assist you with enrollment.

For those without insurance DMCHC accepts payments using a sliding fee scale. This is based on family size and income. Health center staff is glad to help you with eligibility. Phone or utility bills are required for proof of California residency.

Your co-payment is expected at the time of your visit. For your convenience, we accept cash, checks, Debit, VISA and MasterCard. A receipt of payment is given at time of payment for your records.

In order to provide the best care possible, your physician will review your previous medical records if available. Please have all records forwarded to our office from your current health care provider. Mail or bring in person any records you have at home before your first visit.

Please arrive at least 20 minutes early for your first visit to complete patient registration before your appointment. Please bring your insurance cards or other indemnification if you’re without insurance.

DMCHC takes the confidential nature of your health information very seriously. You will be given our Notice of Privacy Practices describing how we use your health information and outlining your privacy rights under HIPAA , the federal regulation governing patient privacy.

We ask you to sign an acknowledgment that you have received the Notice of Privacy Practices. This allows only the individuals listed on the form to access your health information such as test results or copies of health records.
