Sliding Fee is a special program at Darin M. Camarena Health Centers, Inc. (DMCHC) to assist those who are uninsured or have diffculty paying for medical care. Depending on your income and family size, sliding fee scale decides how much you and your family will pay for medical services. At DMCHC, services that qualify for sliding fee scales are medical, dental and laboratory services. Please note that not all procedures may qualify for sliding fee discount.

If you who are not eligible for any insurance coverage (e.g. Medi-Cal, Medicare), and have limited income, you can apply for sliding fee. You pay a fixed fee based on income and family size. In order to verify your income, please bring the following as proof of income:

• Most current payroll check stubs for one month
• Unemployment award letter
• Most current W2’s & 1099’s
• Social Security or SSI award letter or copy of check.
• Ask our friendly health center staff for other forms of proof of income.

You will be responsible to:

• Present all required information for the Sliding Fee Program at the time of service.
• Re-certify every six (6) months providing updated information.
• Provide a payment at the time of service.
