Resumen:

The Licensed Vocational Nurse (LVN) facilitates patient access by overseeing and assisting the Medical Support Manager LVN, and nursing staff for the pod areas.  Additionally, it is the role of the LVN to determine the immediacy of a patient’s health issues when presenting at the health center.  The LVN does this by performing an immediate assessment, and makes decision with protocol guidelines (i.e., febrile regimen, acute asthma symptoms) and or relays pertinent medical information to clinicians to determine appropriate patient care needs.  This licensed position demonstrates initiative in resolving personnel/staff issues with sound judgment and decision-making.  The LVN facilitates the provision of information needed by patients, staff and clinicians through the service delivery.  Additionally, the LVN is responsible for the functions, coordination, and oversight of the health center’s Community Wellness/chronic disease management program.

 

The LVN works closely with the Medical Support Manager LVN, providers, health maintenance specialists, grant personnel and other members of the medical service delivery team.  This position also works closely with the Provider Scheduler to ensure adequate coverage of personnel in the pods and the assignment of patients to balance the overall flow with the health center.

 

The LVN implements changes in procedures according to clinical protocols and guidelines with and under the direction of the Medical Support Manager LVN.  It is the responsibility of the LVN to ensure that service delivery staff are trained adequately and are tested for competency.

Funciones y responsabilidades:

  • Focus on Patient Assessments:

1.1          Supports the goal of quality patient services by assisting medical provider personnel in special patient procedures (i.e. dressing changes, ear irrigations, etc.) as outlined in the Nursing Procedure Manual.

1.2          Supports patient safety and access by performing diagnostic tests with their licensed scope of work and upon order of medical provider and in accordance with established policies and procedures.

1.3          Ensures patient safety by triaging patients in acute situations when presenting in Walk-in Center call-in patients requesting Urgent Care service.

1.4          Supports patient focused outcomes by providing case management oversight to DMCHC projects and programs (e.g., Cancer Detection Program, Title X reporting).

1.5          Meets the goals of the health center by providing oversight and management of the Back Support Team, which includes Chronic Disease Management Project Coordinator, Health Maintenance Specialists, Medical Unit Clerks, and Medical Assistants.

1.6          Supports the team effort by preparing patients on intake to be seen by medical provider.

1.7          Supports DMC’s quality and safety measures by serving as back up to patient complaints, addressing appropriately and timely.  In addition, this position is responsible for investigation and reporting of findings to Medical Support Manager LVN for reporting at the Continual Quality Improvement (CQI) team.

1.8          Supports the overall mission of the health center by providing ongoing staff development of Medical Assistant, Medical Unit Clerk, and Health Maintenance Specialist skill levels.  Provides assessment and competency assessments, plus works with staff to develop individual staff development plans.

2.0  Focus on Medical Support Manager LVN

2.1.         Supports the mission and direction of the health center by assisting the Medical Support Manager LVN in the development and implementation various health maintenance procedures.

2.2          Supports the health center’s compliance efforts by assisting with annual update to Clinical scope of works manuals, tests applicability, and communicates changes with the team consistently.

2.3          Ensures functional equipment maintenance; facilitates annual calibration schedule, monthly review of logs and report to Medical Support Manager LVN on an on-going quarterly basis.
