Position: Community Outreach Coordinator

Responsible To : Director of Community Outreach

Summary: 

The Community Outreach Coordinator will provide administrative, outreach, and service coordination support for Camarena Health. Outreach and service coordination functions focus on engaging students, families, school administrative staff, and the school community in support of general population health to promote Health Center services. This position will work in partnership and joint accountability with team members to achieve Camarena Health’s Mission, Guiding Principles and Goals.

EXPECTATIONS:

  • Arrives on time and adheres to set schedule.
  • Maintains open communication with Director of Community Development and other staff, acting as liaison between the Community and Camarena staff and patients.
  • Understands and educates students on health center services, program services and resources.
  • Works closely with the Director of Community Development, to ensure smooth integration of scope of program.
  • Completes and submits timely reports on assigned scope of work.
  • Maintains cleanliness and order of work area, equipment and supplies.
  • Use of professionalism and best efforts in the position.

Duties and Responsibilities:

  • Focus on Outreach and Health Center Programs:
    • Coordinate and lead targeted outreach activities focused on promoting, integrating and informing patients, school staff, students, and families and the general community about the Health Center services.
    • Coordinate and/or perform basic educational outreach activities focused on health promotion and health education to students and families.
    • Develop and manage materials and announcements associated with Health Center outreach activities.
    • Achieve extensive knowledge base becoming a valuable resource for Outreach Coordinator.
    • Attend Outreach events within Madera County to meet and provide relevant information to potential candidates.
    • Train for and complete State and Federal training requirements.
    • Be proficient in and understand current and new regulations set forth in the ACA.
    • Attends assigned community events by the Director of Community Development.

 

  • Focus on Service Coordination:
    • Serve as the primary point of contact for referrals and service inquiries from the community and families for Health Center services.
    • Attend school meetings related to identification and coordination of health support services for students.
    • If patient needs are outside the scope of Health Center services, educate patients, school staff, and families on other health care and basic support services available in the community and within the school through student support/intervention systems.
    • Other duties as assigned.

    • Attends workshops /seminars as necessary to increase knowledge and keep up with the changes to programs in order to provide effective patient information to enrollment opportunities.
    • Supports the functions of the health center and team by working flexible or extended hours when necessary.
    • Demonstrates awareness of, and compliance with, organizational mission and objective of Camarena Health to provide health care access and support services for all members of the Community.
    • Other work-related duties as assigned by Supervisor. Duties and responsibilities may be added, deleted, or changed at any time at the discretion of management, formally or informally either verbally or in writing.
    • Maintains confidentiality and respect for information regarding patients and other team members; abides by Camarena Health Rules of Confidentiality.

 

 

Minimum Requirements:

Education:                         

  • Bachelors degree in Public Health, Social Work, or related field from an accredited college or university is preferred.

 

Prior Experience:                            

  • Community Outreach of social service preferred.

  • Fully bilingual (English-Spanish);

  • Ability to develop meaningful and appropriate relationships with students, school staff and families at different school sites

  • Demonstrated good problem-solving skills;
  • Intermediate to expert computer skills;
  • Computer proficiency in MS Office, Word, Excel, and Power Point and various software programs;

 

 

Physical Requirements:               

  • Must be able to move up to 25 pounds.
  • Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
  • Must be able to have vision that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
  • Must have high manual dexterity.
  • Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
