You can complete the entire nomination form above. You will be asked to provide two additional names of other employees that can endorse this employee and the great work they do. They will also be sent an email to complete.

Can Someone Win Employee of the Month more than once

No, unfortunately an employee can only win once a year. There are so any great employees at Camarena Health and we want to make sure we have the opportunity to highlight other employees.

What Do the Winners Get?

Winners receive a $50 gift card and a lunch with your supervisor.

Can I Vote for More Than One Person

Yes, you can submit more than one person. Each nominee will require a different form.
