Turning 18 is an exciting milestone. For young adults with medically complex conditions, it can also bring a major change in how they receive care. After years with pediatric providers who know their health history, patients and families may need to find new clinicians, navigate different services, and take on a greater role in managing their health.
That transition takes more than a referral. It takes a plan, trusted relationships, and a team willing to help each young adult through the process.

To help make that possible, Camarena Health partnered with Valley Children’s Hospital to support young adults moving from pediatric specialty care to adult primary care. Through Valley Children’s Adult and Transition of Care program, the teams work together to coordinate care, develop a healthcare transition roadmap, and help patients ages 18 and older explore their options for adult primary care.
For patients and families, that support can make an unfamiliar process feel more manageable. It helps preserve continuity of care while giving young adults the guidance and resources to take the next steps with confidence.
